USCIS Civil Surgeon

Edison Medical Associates is an USCIS designated civil surgeon Office authorized to conduct immigration (inmigracion) medical examinations of aliens for adjustment of status. The office can help immigrants complete their medical examination requirements for their green cards.

Our office offers competitive pricing, conducts Immigration / immigracion medical exam, and provides all the tests and vaccinations needed to complete the I-693 forms in one convenient location.

To complete the medial examinations, following tests may have to be completed.

  • Physical Examination
  • Blood Test for TB (Serum IGRA/ Quantiferon gold) *
  • Blood Test for syphilis
  • Urine for Gonorrhea
  • Vaccinations

*  Starting Oct 1, 2018 as per USCIS guidelines, TB skin test will no longer be provided and will be replaced by the blood test for TB

Pricing $345 includes physical exam, blood work for Tuberculosis (Serum Quantiferon Gold / IGRA) Syphilis and urine test for Gonorrhea and completing the paperwork as per USCIS requirements.

This cost does NOT include any vaccinations / chest X ray  to satisfy the USCIS requirements.  Any additional vaccination required can be provided at the office for an additional cost.

WHAT TO EXPECT

On the first day of visit, physical exam is conducted and blood test for TB and urine specimen will be collected. The civil surgeon will review the applicant’s vaccination record and notify the vaccinations required to complete the form I-693. If no prior record of immunization is available, then those required can be conveniently administered at the office.

If the blood test for TB comes back positive, you will be required to get a chest X-ray to complete the process.

You will receive the completed I-693 form, in a sealed envelope in 2 to 3 days.

DOCUMENTS THAT WILL ASSIST

  • Download and print and fill  the latest version of I-693 forms, else , the USCIS will reject
  • Govt issued photo ID (passport or drivers license)
  • Vaccination record (if available)
  • Most adults 18 years to 64 years require proof of Td/Tdap, MMR vaccine (Measles, mumps, rubella) and Varicella (chicken pox). Flu shot is required in flu season (Oct to March)
  • Record of prior TB testing (especially if positive)
  • I-693 application can be downloaded and part I can be filled by the applicant. Do not sign until asked to do so by the civil surgeon. If you have an alien number, it can be filled in.

 

We provide all tests as mandated by USCIS to complete the I-693 form in our office for your convenience. In addition to being a convenient one-stop service, you will receive your sealed envelope within 2-3 business days.
